This script is how you add a mirror of the CECID PHP script to our database. Fill in the mandanatory fields,
and fill in the optional email, if possible. When you click "Add mirror" an email will be sent to a site
admin, who will verify your request and add you to the database. Your email will allow us to notify you that
your mirror has been added, or if there is any problems with the mirror in the future. If there are problems,
and you havnt left a contact email, your mirror will simply be removed from the database. Your email will
most definantly NOT be shared with third parties, and will only be used for the purposes stated above.
